stacjo Terms of Service

Draft document. Legal approval is required before publication.

1. General provisions

stacjo is provided by [NAZWA SPÓŁKI], tax ID [NIP], register number [KRS], at [ADRES], email [E-MAIL KONTAKTOWY].

These terms govern use of stacjo by the entity running a practice, called the practice below.

The practice is the party to the stacjo agreement. A patient who uses online booking or the patient portal does not become a party to that agreement.

These terms are available free of charge before the agreement is made and can be saved and reproduced.

2. Scope of service

stacjo gives the practice a calendar, patient records, notes, documents, email and SMS reminders, a patient portal, team roles and online bookings.

Billing features are available from the Gabinet plan.

The price list sets out each plan's features, limits and prices. Prices exclude VAT, which is added where applicable.

The free trial lasts 7 days. Sending SMS may require an available allowance or an SMS packet.

3. Agreement and account

A person authorized to represent the practice creates an account and provides practice details. stacjo reviews the request manually. Sending a request does not grant access to a practice account. The agreement is made when stacjo approves the request and grants access to the practice account.

The practice keeps its details current, gives team members access based on their roles and protects login credentials. Users must not share accounts with unauthorized people.

4. Technical requirements and use

Using stacjo requires internet access, an up-to-date browser that supports JavaScript and cookies needed for login, and an active email address.

The practice must not upload unlawful content or infringe the rights of others.

The practice is responsible for data entered by its team, the legal grounds for processing it and its duties to patients. stacjo is responsible for providing the service under the agreement and applicable law.

5. Online bookings

On the practice's booking page, a patient selects a service, a person and a time, then sends an appointment request. The time is not confirmed when the request is sent. The practice confirms or declines the request. After the practice confirms the request, the patient receives a confirmation email.

The practice is responsible for its services, available times and terms for patients. stacjo provides the tool used to handle the request.

6. Payments and free trial

The free trial lasts 7 days. Before ordering a paid plan, the practice sees the net price, VAT, billing period and limits. Payment terms for plans and SMS packets are shown before the order.

If the practice does not choose a paid plan, it can view but not edit its data for 14 days after the trial ends. Access to the account is then blocked.

7. Data and processing agreement

The practice decides why and on what legal grounds it processes patient data, including health data. For that data, stacjo acts as a processor on the practice's documented instructions. A separate data processing agreement sets out the terms and must be concluded before patient data is entered. Contact [E-MAIL KONTAKTOWY] about the agreement.

stacjo is the controller for data needed to run the practice account, handle payments, secure the service and meet its own legal duties. The Privacy Policy gives more detail.

8. Security and interruptions

stacjo applies security measures suited to the risks of processing the data. It may limit access temporarily for maintenance, failures or security.

The practice keeps its own continuity procedures and does not rely only on system availability for urgent patient care. This clause does not exclude any liability that cannot be excluded by law.

9. Complaints

The practice may send a service complaint to [E-MAIL KONTAKTOWY]. It should describe the issue and provide details needed to find the account. It should not send patient health data through this channel. stacjo acknowledges receipt and replies without undue delay.

10. Changes and termination

stacjo may change these terms for a valid reason, such as a change in law, service features or security requirements.

Before a material change takes effect, it provides the text and effective date and tells the practice how to end the agreement if it does not wish to continue. Changes do not apply retroactively. The practice may give notice to end the agreement by emailing [E-MAIL KONTAKTOWY].

11. Liability and governing law

The practice is responsible for care decisions and for keeping patient records as required by law. stacjo is responsible for its own service and data protection duties. Nothing in these terms limits rights that cannot be excluded, including rights of a sole trader when the agreement is outside their professional activity. Polish law applies. The court with jurisdiction under applicable law will hear disputes.

12. Contact

Questions about these terms can be sent to [E-MAIL KONTAKTOWY].
